Need a better way to track IEP Goals? This Google Form will collect data for a Mathematics IEP Goal. It is formatted specifically for a student that will use their math resources and notes before asking for assistance on independent work. Easily customize this form with specifics or use it as is. Share it with the student's other teachers, aides, and other service providers so that all of the data can be populated into the Google Sheet generated from the form's responses, and can be held in one place! You can also set the form to automatically email you every time it is filled out! I find this is so helpful as a case manager of HS students (since I am not with my students in every single class)! It can also help me address whether or not a colleague is filling out their part of the paperwork, instead of being surprised and scrambling at Progress Report time. This data collection form is designed to meet your needs when using Teaching Strategies Gold (TSG) for preschool. Log your observational notes to later record as documentation for your pre-k students. A wide rating skill was provided to accommodate early childhood special education. Once you’ve typed all of your students’ names onto the first page, they will generate onto each data sheet. There is one objective (with ratings from 2-8 or 1-6 depending on how the skill matched with age bands) on every sheet. 10 student names can be printed on one page of the data sheet. Do you have more than 10 students per class? Yep, me too! I just print double-sided to ensure that each session is on one sheet. The document is all set to accommodate up to 20 students per session. (There are only 10 student rows to ensure that there is enough room for note-taking.) Assessing with TSG? You need this data sheet, trust me! I’ve tried it several other ways. I provide much more skill specific information when I have the rating scale along with the objectives listed. This is also a BIG help to paras collecting data. Data collecting, analyzing, and drawing conclusions are important skills for our students to learn, but it doesn't have to be boring! Let students use the Google apps they love to survey their classmates on topics that interest them. Teach them how to collaborate with each other. Let them watch their survey results roll in in real time. Lead them to think deeply about the data they've collected and let them make charts, graphs and digital presentations to share the conclusions that they've drawn.
